Dr. Aaron J. Duberstein, M.D.
Dr. Aaron J. Duberstein is a native of Memphis, Tennessee.  He completed his undergraduate work at University of Southern California in 1999 earning dual degrees in Psychology and Biology.  He attended medical school at University of Tennessee graduating in 2003.  During medical school he was awarded a Maimonides Fellowship for the study of medical ethics.  After training in Houston, Texas for two years he completed his Otolaryngology-Head and Neck Surgery residency in Detroit, Michigan at Wayne State University in 2008.  During his residency Dr. Duberstein was awarded a Lions Club Grant for his research in biofilm mediated disease in the head and neck.  He then spent additional time training in facial plastic and reconstructive surgery in the Indianapolis area before going into private practice.  Dr. Duberstein is a member of the American Academy of Otolaryngology-Head and Neck Surgery and is Board Certified by the American Board of Otolaryngology.  He enjoys all aspects of Otolaryngology and has specific interests in disorders affecting the pediatric population, allergic and infectious diseases of the nose and sinuses, and sleep medicine.  Outside of the office, Dr. Duberstein enjoys spending time with his wife and daughter, running, and outdoor sports.

Dr. Peter J. Hillsamer, M.D.
Dr. Peter J. Hillsamer, a native of Michigan City, Indiana, completed his undergraduate work at Indiana University in 1981 and Indiana University Medical School in 1985. After Medical school he served one year of general surgery at Riverside Methodist Hospital in Columbus, Ohio. He then received specialized training in Otolaryngology and Facial Plastic and Reconstruction Surgery at Ohio State University from 1986 to 1990.

Dr. Hillsamer is Board Certified by the American Board of Otolaryngology-Head and Neck Surgery.

Dr. Daniel R. Berner, M.D.
Dr. Daniel R. Berner, a native of Kalamazoo, Michigan, completed his undergraduate work at Alion College in 1972.  He graduated from the University of Michigan School of Medicine in 1976.  After medical school he served a two year general surgery residency at Butterworth Hospital in Grand Rapids, Michigan.  He then received specialized training in Otolaryngology and Facial Plastic and Reconstructive Surgery at Indiana University Medical Center from 1978 to 1981.

Dr. Berner is Board Certified by the American Board of Otolaryngology-Head and Neck Surgery, a member of the American Academy of Otolaryngology and a Fellow of the American College of Surgeons.

Jonathan B. Shook, MD
Dr. Jonathan Shook graduated with distinction from Purdue University, West Lafayette, Indiana in 1998. In 2002, he received his medical degree from Indiana University School of Medicine, Indianapolis, Indiana. During medical school, he was elected to the pres¬tigious medical honor society, Alpha Omega Alpha, based on his high academic achievement.

In 2007, Dr. Shook completed his residency in Ortho¬paedic Surgery at Loyola University Medical Center in Maywood, Illinois. During his residency he received several awards, including: Innovations in Orthopaedics Award, Certificate of Excellence in Research and the Charles M. Schwartz Traveling Fellow Award. He completed a Sports Medicine fellowship at the University of Chicago Hospitals in 2008.

Dr. Shook is a member the American Academy of Orthopaedic Surgeons, American Orthopaedic Society for Sports Medicine and the Indiana Orthopaedic Society.

Dr. Shook is currently head team physician for West¬field High School and a part of the medical team for the Indianapolis Indi¬ans and the Indiana Fever of the WNBA. He was the official orthopaedic consultant for the 2010 and 2011 NCAA Final Four Tournament. Dr. Shook has served as a team physician at the University of Chicago. He was also a member of the medical team involved in the treatment of the Chicago Blackhawks of the NHL.

Dr. Shook’s work has been published in multiple professional journals.

James D. (Sam) Baldwin, M.D.

Dr. Baldwin has been practicing in Lafayette since 2002. A native of Salt Lake City, Utah, he received both his undergraduate and medical degrees at the University of Utah. He completed a general surgery residency at the University of Rochester, and finished his plastic surgery training at the University of Utah. He is formerly an Assistant (Clinical) Professor at the University of Utah Medical Center and has participated on the teaching faculty in the Division of Plastic Surgery at the University of Rochester. Dr. Baldwin is board certified by the American Board of Plastic Surgery and is a member of the American Society of Plastic Surgery.

Dr. Baldwin and his wife, Cindy, have 4 children.
