In 2007, Dr. Shook completed his residency in Ortho¬paedic Surgery at Loyola University Medical Center in Maywood, Illinois. During his residency he received several awards, including: Innovations in Orthopaedics Award, Certificate of Excellence in Research and the Charles M. Schwartz Traveling Fellow Award. He completed a Sports Medicine fellowship at the University of Chicago Hospitals in 2008.
Dr. Shook is a member the American Academy of Orthopaedic Surgeons, American Orthopaedic Society for Sports Medicine and the Indiana Orthopaedic Society.
Dr. Shook is currently head team physician for West¬field High School and a part of the medical team for the Indianapolis Indi¬ans and the Indiana Fever of the WNBA. He was the official orthopaedic consultant for the 2010 and 2011 NCAA Final Four Tournament. Dr. Shook has served as a team physician at the University of Chicago. He was also a member of the medical team involved in the treatment of the Chicago Blackhawks of the NHL.

Dr. Baldwin has been practicing in Lafayette since 2002. A native of Salt Lake City, Utah, he received both his undergraduate and medical degrees at the University of Utah. He completed a general surgery residency at the University of Rochester, and finished his plastic surgery training at the University of Utah. He is formerly an Assistant (Clinical) Professor at the University of Utah Medical Center and has participated on the teaching faculty in the Division of Plastic Surgery at the University of Rochester. Dr. Baldwin is board certified by the American Board of Plastic Surgery and is a member of the American Society of Plastic Surgery.
Dr. Baldwin and his wife, Cindy, have 4 children.
